Section R9-33-102. Requirement for Licensure

A.      An applicant shall obtain a license to operate a group home from the Department before providing supervision or habilita- tion to an individual with a developmental disability in a group home.

B.       A license to operate a group home is valid for the following, as indicated on the license:

1.        Address of the group home;

2.        Name of the licensee;

3.        Name of the group home, if applicable;

4.        Fire risk prevention level; and

5.        Licensing period for the group home.
