Section R9-23-203. Participation Requirements

The contact person for a participating school shall ensure that each child participating in the Arizona Dental Sealant Program has on file at the school a parental consent form provided by the Depart- ment that includes:

1. The child’s name, and

2. A parent’s signature indicating permission to participate in the Arizona Dental Sealant Program.
