  • A.      After the Department or the Department’s designee determines that a violation of A.R.S. § 36-601.01 has occurred, and based on the criteria in R9-2-112, the Department or the Depart- ment’s designee may send to the proprietor at the place of employment or public place a written notice of violation that includes:

    1.        The nature of the violation;

    2.        The date and time that the violation occurred;

    3.        The name, telephone number, and e-mail address of the Department contact person or the contact person of the Department’s designee; and

    4.        If a civil penalty is being assessed, a notice of assessment.
