  • A.      Within six months after receiving the written notice in R9-16- 303(E), an applicant for an initial license by examination shall submit to the Department:

    1.        An application in a format provided by the Department that contains:

    a.         The applicant's name, home address, telephone num- ber, and e-mail address;

    b.        An attestation that the information submitted as part of the application for approval to take the Depart- ment-designated written hearing aid dispenser examination required in R9-16-304 is currently true and accurate; and

    c.         The applicant’s signature and date signed; and

    2.        A license fee of $200.

    B.       The Department shall review an application for an initial hear- ing aid dispenser license by examination according to R9-16- 316 and Table 3.1.

    C.      If the Department does not issue an initial hearing aid dis- penser license by examination to an applicant, the Department shall return the license fee to the applicant.

    D.      An initial hearing aid dispenser license is valid for two years from the date of issue.
