Section R9-16-116. Denial, Suspension, or Revocation of License; Civil Penalties; Procedures

  • In addition to the grounds specified in A.R.S. §§ 36-756 and 13- 904(E), the Department may deny, suspend, or revoke a license per- manently or for a definite period of time, and may assess a civil penalty for each violation, for any of the following causes:

    1.        Practicing under a false name or alias so as to interfere with or obstruct the investigative or regulatory process,

    2.        Practicing under the influence of drugs or alcohol,

    3.        Falsification of records,

    4.        Obtaining any fee for midwifery services by fraud or mis- representation,

    5.        Permitting another to use the midwife's license, or

    6.        Knowingly providing false information to the Depart- ment.
