iv.       Soap for hand washing,

v.        Paper towels or a mechanical air hand dryer,

vi.       Lighting, and

vii.     A means of ventilation; and

b.        Is for the exclusive use of the health care institution;

3.        A pest control program is implemented and documented;

4.        If pets or animals are allowed in the health care institu- tion, pets or animals are:

a.         Controlled to prevent endangering the patients and to maintain sanitation;

b.        Licensed consistent with local ordinances; and

c.         For a dog or a cat, vaccinated against rabies;

5.        A smoke-free environment is maintained on the premises;

6.        A refrigerator used to store a medication is:

a.         Maintained in working order, and

b.        Only used to store medications;

7.        Equipment at the health care institution is:

a.         Sufficient  to  provide  the  health  care  institution’s scope of service;

b.        Maintained in working condition;

c.         Used according to the manufacturer's recommenda- tions; and

d.        If applicable, tested and calibrated according to the manufacturer’s recommendations or, if there are no manufacturer’s recommendations, as specified in policies and procedures;

8.        Documentation of an equipment test, calibration, and repair is maintained for at least 12 months after the date of testing, calibration, or repair; and

9.        Combustible or flammable liquids and hazardous materi- als stored by the health care institution are stored in the original labeled containers or safety containers in a stor- age area that is locked and inaccessible to patients.
