  • In addition to the license application requirements in A.R.S. § 36- 422 and R9-10-105, an applicant for a license as a home health agency shall:

    1.        Include on the application:

    a.         The  name  and  address  of  each  proposed  branch office, if applicable; and

    b.        The geographic region to be served by:

    i.         The proposed home health agency’s adminis- trative office, and

    ii.        Each proposed branch office; and

    2.        Submit to the Department a copy of a valid fingerprint clearance card issued according to A.R.S. Title 41, Chap- ter 12, Article 3.1 for:

    a.         The applicant, if the applicant is an individual; or

    b.        Each individual with a 10% or greater ownership of the business organization, if the applicant is a busi- ness organization.
