Section R9-10-1108. Care Plan

An administrator shall ensure that a care plan for a participant:
1. Is developed within seven calendar days after the comple- tion of the participant’s comprehensive assessment;
2. Has input from:
a. The participant or participant’s representative,
b. The registered nurse who performed the comprehen- sive assessment, and
c. Personnel who have provided services to the partici- pant;
3. Is based on the participant’s comprehensive assessment;
4. Includes:
a. A summary of the participant’s medical or health problems, including physical, mental, and emotional disabilities or impairments;
b. Adult day health services to be provided;
c. Goals and objectives of care that are time-limited and measurable;
d. Interventions required to achieve objectives, includ- ing recommendations for therapy and referrals to other service providers; and
e. Discharge instructions according to R9-10-1109(B); and
5. Is reviewed and updated at least once every six months and whenever there is a significant change in the partici- pant’s condition.
