Section R9-10-1002. Supplemental Application Requirements

c.         Physical health services;

d.        Rehabilitation services;

e.         Sleep disorder services; or

f.         Urgent  care  services  provided  in  a  freestanding urgent care center setting.

B.       In addition to the license application requirements in A.R.S. § 36-422 and 9 A.A.C. 10, Article 1, a governing authority of an affiliated outpatient treatment center, as defined in R9-10- 1901, applying for an initial or renewal license for the affili- ated outpatient treatment center shall submit, in a format pro- vided by the Department, the following information for each counseling facility for which the affiliated outpatient treatment center is providing administrative support:

1.        Name, and

2.        Either:

a.         The license number assigned to the counseling facil- ity by the Department; or

b.        If the counseling facility is not currently licensed, the:

i.         Counseling facility's street address, and

ii.        Date the counseling facility submitted to the Department an initial application for a health care institution license.
