  • A.      A licensee shall report a lost or stolen license to the Board, in writing or electronically through the Board website, within 30 days of the loss.

    B.       A licensee requesting a duplicate license shall file an applica- tion on a form provided by the Board for a duplicate license and pay the applicable fee under A.R.S. § 32-1643(A)(14).
