Section R4-11-205. Application for Dental Assistant Radiography Certification by Credential

A.      An applicant for dental assistant radiography certification by credential shall provide to the Board a completed application, on a form furnished by the Board that contains the following information:

1.        A sworn statement of the applicant’s eligibility, and

2.        A letter of endorsement that verifies compliance with R4- 11-204.

B.       Based upon review of information provided under subsection (A), the Board or its designee shall request that an applicant for dental assistant radiography certification by credential pro- vide a copy of a certified document that indicates the reason for a name change if the applicant’s documentation contains different names.
