Section R3-11-707. Application for a Veterinary Medical Premises License

  • An applicant for a veterinary medical premises license shall:

    1.        Submit the following to the Board:

    a.        A notarized application form, signed by the Respon- sible Veterinarian, that contains the information set forth in A.R.S. § 32-2272; and

    b.        The fee required in R3-11-105, payable by certified check or money order; and

    2.        Pass an inspection conducted by the Board.
