Section R3-11-1010. Change in Operator

Within 30 days after a change in operator, the Responsible Owner shall provide a written notice to the Board that includes:
1. Name of the licensed animal crematory;
2. Animal crematory license number;
3. Name of the former operator;
4. Name of the new operator;
5. Date on which the new operator assumed responsibility for the animal crematory; and
6. An affirmation, signed by the Responsible Owner, that the new operator received training in the safe and proper operation of the cremation chamber and the written pro- cedures required under R3-11-1007.
