  • A.      Except as provided herein, there are no specific requirements as to the form of any pleading. A pleading is simply required to contain a caption sufficient to identify the parties in accor- dance with R20-5-812, which shall include the Commission’s citation number, and a clear and plain statement of the relief that is sought, together with the grounds therefor.

    B.       Pleadings and other documents (other than exhibits and peti- tions for hearing) shall be typewritten and double spaced, on letter size opaque paper (approximately 8 1/2 inches by 11 inches). The left margin shall be 1 1/2 inches and the right margin 1 inch. Pleadings and other documents shall be fas- tened at the upper left corner.

    C.      Pleadings shall be signed by the party filing or by his represen- tative. Such signing constitutes a representation by the signer that he has read the document or pleading, that to the best of his knowledge, information and belief the statements made therein are true, and that it is not interposed for delay.

    D.      The Commission may refuse for filing any pleading or docu- ment which does not comply with the requirements of subsec- tions (A), (B), and (C) of this Section.
