Section R2-7-F301. Statement of Qualifications

A. The agency chief procurement officer may request that per- sons desiring to provide the services specified in A.R.S. § 41- 2513 submit statements of qualifications on a prescribed form which shall include, but not be limited to the following infor- mation:
1. Technical education and training;
2. General or special experience, certifications, licenses, and memberships in professional associations, societies, or boards; and
3. Any other relevant information requested by the purchas- ing agency.
B. Persons who have submitted statement of qualifications may submit additional information or change information that was previously submitted at any time.
C. The agency chief procurement officer may, in lieu of subsec- tion (A), incorporate the statement of qualifications as part of the solicitation pursuant to R2-7-F302.
