  • A.      Any person who would like to hold a special event may apply for a permit by filing, either in person or by mail, a Depart- ment-approved event application form with the Office of Spe- cial Events.

    B.       The completed application form shall be submitted at least two days before the desired date of the special event. A completed application form is one that is legible and contains, at a mini- mum, all of the following information:

    1.        The name, address, and telephone number of the sponsor;

    2.        The proposed date of the event and the approximate start- ing and concluding times;

    3.        The specific, proposed location for the event;

    4.        A general description of the event, including equipment and facilities to be used;

    5.        Approximate number of persons expected to be in atten- dance;

    6.        The name, address, and telephone number of the person responsible for clean-up of the area after the activity, if different from the person in subsection (B)(1);

    7.        The name, address, and telephone number of any chief monitor who will be designated to direct the event;

    8.        A description of the badge or article of clothing used to identify monitors;

    9.        A copy of any insurance policy for the special event; and

    10.     A copy of any contract for medical, sanitary, and security services.

    C.      The Director may accept a completed application form submit- ted less than two days before a press conference if the Director determines that enforcing the two-day requirement would nul- lify the need for the press conference. In this situation, R2-11- 404 does not apply.
