  • A.      A licensee or traffic survival school qualified instructor, mak- ing or learning of any change in the content of its original application information, other than ownership, shall provide written notification of the change, completed on a form approved by the Department and signed by a principal or busi- ness manager, to the Department or private entity within two business days of making the change.

    B.       A licensed school making a change to a principal or corporate structure shall submit to the Department or private entity a new application for licensing under this Article and all appli- cable fees, as a new applicant for licensure, within 10 calendar days of making the change.

    C.      A licensed school submitting a new application to the Depart- ment or private entity, as provided under subsection (B), is subject to the fingerprint clearance card requirement under

    R17-5-304 unless a valid fingerprint clearance card is already on file with the Department.

    D.      A licensed school shall provide written or electronic notifica- tion on a form, approved by the Department, to the Depart- ment or private entity within 10 calendar days of making any changes to the licensee’s contact person, business manager, or instructors.
