Section R13-2-306. Change in Name of Registrant

A registrant whose name has changed shall notify the Department in writing within 30 days of the name change and may request a new identification card. If the registrant comes to the Department in person, the registrant shall present to the Department a government- issued photo identification card with the new name or court docu- ments recording the name change and the fees under R13-2-102. If the registrant sends a request by mail, the registrant shall mail to the Department certified, notarized copies of any court documents with a 1" x 1 1/4" inch photo for the identification card photo and the applicable fee under R13-2-102.
