Section R10-4-504. Reports

Within 15 days after the end of each calendar quarter, a grantee shall submit a written report, on a form prescribed by the Commis- sion, containing:
1. A financial report that includes itemized budget informa- tion, and
2. An activity report that documents activities supported by the grant funds and includes:
a. A narrative of activities undertaken during the reporting period;
b. An evaluation of progress toward achieving the goals and objectives in the grant application;
c. An evaluation of adherence to the time-frames in the grant application; and
d. A description of equipment purchased with grant funds during the reporting period, how the equip- ment is related to achieving the goals and objectives of the project, and the current status of the equip- ment, such as whether it is operational, waiting to be installed, or undergoing testing; and
3. A copy of any deliverable provided by a consultant paid with grant funds.
